How to Create Your Own Form for the Self Certification Check
On package set up or the package editor, you will need to toggle **on** the self-certification check.
When you create a package, you will only see the I-9 Form option but **if you need to create your own form, you can only do this by editing the package once the package is created**.

Please note, additionally, **you can only make one form per package**. We don’t have the ability to have multiple forms on a package.
# Create your own form
1. Select your desired package with the Self Certification turned on for on the **Packages** page.
2. Once in your package editor, select **Create your own form**.
3. Choose how you want to create your custom form:
- Select **Start from scratch** to create your own custom form
- Select **Use a template** to start from a pre-created template
With both of these, you can create, edit questions, and add in your own acceptance criteria as you see fit.

## Use a template
If you select **Use a template**, we currently have 3 templates available.
- FCA Senior Managers F&P
- FCA Certified Persons F&P
- Fitness and Propriety

These are all commonly used in the finance industry. We will build out more Self Certification templates in the future.
Select which template you would like to use.
The chosen template will now add all of the questions from the template into the form builder. You are now free to edit the questions and add your own acceptance criteria to any questions.
Don’t see a question you wanted to ask? Add it wherever you need it to be placed!
## Start from scratch
When you select to create your own questionnaire from scratch, you will be taken to the form builder.
Here, you can name your form and add in an optional form description such as directions for the candidate to use to complete this section.
You will also be able to choose from 4 question types:
- Yes/no + conditional
- Text to ask open-ended questions
- Multiple choice
- Scale (1-5)

### Yes/No
In the Yes/No questions, you can set the question as optional or mandatory by toggling on or off required in the top right corner.
You can then add your question and give it a description if needed with some extra information like defining what Yes means
Finally, you can set an acceptance criteria where based on the answer being Yes and/or No, the result will be **Clear**.
For example, if you want to ask the question “do you require any additional support in this role?”, with the answer No, you can set this as **Accept as clear if the answer is No** so only those that answer Yes are flagged for review.
There is also an option for either answer to be flagged as **clear** in case you don’t need to review any answer provided for this question type.
You can also add **Ask follow-up** where if the candidate answers a certain way to the initial question, you can ask another question as follow up.

### Multiple choice
For multiple choice questions, you can set the question as optional or mandatory by toggling on or off required in the top right corner.
You can then add your question and give it a description if needed.
Specifically to the multiple choice question type, you can input multiple different options and allow candidates to select multiple options for a question asked.
You can select multiple different answers as **Clear** if the candidate selects them.

### Scale
For scale questions, the same as above with setting the question as optional or mandatory and the ability to add in a description if needed.
Here, you can also input what each end of the scale is defined so it’s clear to candidates what they should input and customise it.
Additionally, you can select a range of options to accept as **Clear** so if you want to review anyone that submits 4-5, you would input the range to accept as Clear as 1-3.

### Text
Finally with text, there’s no changes!
Most importantly, **there is no ability to set an acceptance criteria**.

# Save and send
Select the question types that are best suited for your Self Certification check and after adding in all your questions, click **Save questions** to save your new form.
Once this form is saved, go back to your package editor and click **Save check package**.
You are now ready to send checks out to candidates using your custom form on your Self Certification check.
